Planning well in advance

If you believe a long term care setting may be needed, it is best to begin planning well in advance. Try to involve the prospective resident in the planning process as much as you can. If the individual has questions regarding care that you are not able to answer, be sure to ask the facility's staff. Remember, experienced, professional caregivers understand both the concerns of the resident and those of the family and are eager to help make the transition as calm and positive as possible.

Services

Assess patients and provide care that patients' physicians have ordered. This includes performing prescribed medical treatment, administering prescribed medications, assessing a chronic illness in which the client’s condition has deteriorated; and teaching treatment, health measures and disease processes. Heartland’s nurses assist in restoration of functions impaired through illness, explaining and making referrals to community resources.

Beyond Basic Care

Physical Therapy

Physical therapy specializes in the evaluation and treatment of problems of the bones and joints as well as the nerves and muscles that move them. Physical therapists perform tests and evaluations, which provide information about joint motion, flexibility, strength and endurance of the muscular skeletal system, postural deviation and fluency of movement relating to a patient’s functional ability.


Treatment methods may include exercise, stretches, massage, neuromuscular facilitation, electrical stimulation, hydrotherapy, cervical or pelvic traction, and a variety of therapeutic heat and cold treatments
.

Occupational Therapy

Occupational therapists use their knowledge of the function of the human body, effects of illness and injury, and the components of activity to increase a patient’s productive activity and assist the patient in achieving full function. By directing a patient’s participation in selected tasks, the therapist can help the patient enhance performance, restore function, diminish or correct pathology, and promote and maintain health. Through the benefits of occupational therapy, the individual can be restored to a level of self-satisfaction and productive living.

Speech Therapy

Communication and safe decision-making are necessary for a normal happy life. Speech, language, cognitive or voice disorders may affect learning, personal relationships, independence and emotional well-being.


A speech, language or voice disorder interferes with an individual’s ability to understand and express thoughts, or produce correct sounds. Cognitive impairments, frequently from a stroke or head injury, may affect an individual’s memory and judgment to make safe decisions in activities of daily living. Swallowing disorders frequently occur after brain injury such as a stroke. Malnutrition, dehydration, weight loss or other medical complications such as aspiration pneumonia may result from impaired chewing and swallowing.


Disorders vary in severity and may affect anyone at any age. Early identification and treatment of speech, language, cognitive, voice and swallowing disorders improves the prognosis for improvement.

Going Home!

We provide discharge planning for our rehab residents that will be returning home. We help provide home care. Physical Therapy services and equipment that may be needed.

We also help to get residents approved for Medicaid or Medicare services.
